The Northwest Territories Health and Social Services Authority –Stanton Territorial Hospital (NTHSSA) is offering challenging Social Work and work life balance. Join our health care team as a Clinical Social Worker. Stanton Territorial Hospital (STH) is a 100 in-patient bed accredited facility, located in Yellowknife Northwest Territories (NWT). STH is the referral center for approximately 43,000 NWT residents and 5,900 residents from the Kitikmeot Region of Nunavut. STH provides health care services to adults and children on an inpatient/outpatient and outreach basis in order to restore health with dignity. STH maintains a tobacco free environment within the building and throughout the property, recognizing the health hazards associated with tobacco in the workplace, both to smokers and non-smokers alike. Reporting to the Clinical Social Worker & Team Leader, Transitional Care Planning (TCPT) Team, this position will provide assessment, counseling, intervention, treatment and discharge planning to individuals presenting with mental health, addictions issues, including those presenting with psychiatric issues under the Mental Health Act to Stanton Territorial Hospital. Clients referred to the incumbent will present a wide range of emotional and mental health or related issues. The incumbent will be required to exercise a significant degree of professional independence in their work with these clients.
The position will be located at Stanton Territorial Hospital to ensure support to clients navigating the processes of the Mental Health Act legislation who are transitioning from impatient psychiatric services to community-based services affiliated with the Mental Health Act.
The Clinical Social Worker position is designed to support the continuum of mental health programming associated with the Mental Health Act, including new initiatives such as assisted community treatment, case management, discharge planning, and improving the continuity of care for patients with mental disorders across the NWT. This territorial service will have strong linkages with NWT-wide community-based mental health programs, psychiatry and programs for immediate and ongoing treatment.
The incumbent is part of the Transition Care Planning (TCP) team as well as part of several multi-disciplinary teams that could include nurses, physicians, rehabilitation counselors and other stakeholders, from all regions of the NWT. While the primary focus of the Clinical Social Worker will be to target Mental Health clients within the Psychiatric Unit, the incumbent may be required to provide service to Mental Health clients admitted to other care areas within STH (including but not limited to Pediatrics, Medicine Unit, Emergency Department), ensuring that requirements identified under the new Mental Health Act are met. In this context, the incumbent will provide assessment, consultation, treatment and service management for individuals with a broad range of mental health issues, and with community partners, support the families of individuals with mental disorders.
The incumbent, in collaboration with the mutlidisciplinary teams, will ensure the assessment of clients, provide interventions in consultation with teams, and ensure that comprehensive and person-centered discharge plans are in place for clients with psychiatric concerns. This position is part of a TCP team that will act as a liaison between hospital and community-based mental health and addictions agencies/services and provides support for system navigation through direct involvement in the discharge plan implementation. One key responsibility is to improve discharge outcomes through a variety of direct interventions and by acting as a contact for discharge information and options for individuals involved in the Mental Health Act.
Typically, the above qualifications would be attained by:
- A Master of Social Work is required;
- Able to demonstrate they are registered as a social worker in the NWT;
- Minimum of 5 years current working experience in mental health services, whether acute or community based; and
- A thorough awareness of NWT regional, community-based and psychiatric resources would be an asset.
Transfer Assignments may be considered.
The successful candidate will be required to obtain proof of current up-to-date immunization status.
Equivalent combinations of education and experience will be considered.
The salary for this position starts at $42.75 per hour (approximately $83,363 per annum) plus an annual Northern Allowance of $3,450
